To improve the health outcomes and daily lives of individuals in the United States with the most complex care needs, five foundations—The Commonwealth Fund, The John A. Hartford Foundation, Robert Wood Johnson Foundation, Peterson Center on Healthcare, and The SCAN Foundation—are working together to accelerate health system transformation. The Playbook, developed by the Institute for Healthcare Improvement and informed by over 150 peer-reviewed journal articles, case studies, step-by-step guides, and practical guidance, explores the challenges facing adults with complex health and social needs and provides evidence-based or experience-informed ideas about how to address those challenges. This webinar taught us more about the development and evolution of The Playbook along with five priorities that funders may consider as they explore opportunities to improve care for people with complex health and social needs. Speakers included Marian Bihrle Johnson, Kedar Mate, and Catherine Mather of the Institute for Healthcare Improvement and John Feather of Grantmakers In Aging.
