Accountable Health Communities

When communities and clinical care organizations come together to address health-related social needs, the result can be improved health outcomes for patients and reduced costs for the health care system. This webinar discussed the “accountable communities of health” model, examples of projects that embrace it, and how its development and implementation is being supported by both public and private funding. Speakers included: Alexander Billioux of the Center for Medicare and Medicaid Innovation, Jeffrey Levi of the Milken Institute School of Public Health at the George Washington University, and Barbara Masters of the California Accountable Communities for Health Initiative.
