People with complex health and social needs—many of whom face challenges such as chronic conditions, behavioral health issues, food insecurity, homelessness, and a history of trauma—often encounter a health care system that is poorly coordinated, inefficient, and expensive.

An Invitation to Innovation: Better Grantmaking, Better Care, Better Outcomes for Vulnerable Populations and Communities was an immersive, one-day, funders-only workshop offering an important opportunity to explore evidence-based innovations and models of care that improve health equity and champion system transformation addressing critical social determinants of health.

The program served as an opportunity for program staff from foundations of any size to strengthen their grantmaking on behalf of vulnerable and hard-to-reach populations, including youth, older adults, LGBTQI people, low-income communities, immigrants, and people of color. Funders heard from leading experts and experienced practitioners, with plenty of time for interactive sharing and learning.
