Are you investing in community collaboration in ways that have been tested, informed, and sustained? Active Living By Design’s Community Action Model evolved from work with hundreds of communities and dozens of funders. Funders who incorporated the model into their health strategies have identified successes, failures, and how they encourage residents and grantees to lead, in order to deepen impact and address health equity. Their work within this model illustrates a process for community-led change, honors community context, defines six essential practices, and demonstrates expected impacts.
