DESCRIPTION:Type 2 diabetes rates continue to skyrocket across the United States\, and people of color\, low-income individuals\, rural communities\, and older adults remain disproportionately affected by the disease. This webinar highlighted lessons learned and promising practices from Together on Diabetes\, an initiative designed to reduce disparities in type 2 diabetes through strategic partnerships\, community-based supportive services\, and patient-centered models of care.
