DESCRIPTION:Grantmakers have made major strides in addressing social determinants of health such as housing\, access to food\, and economic security. As the field deepens its understanding of the root causes of unequal health outcomes\, a growing number of funders are working to bring upstream thinking to hospitals and health systems. This webinar shared how foundations are supporting initiatives that bridge the clinical and nonclinical; integrate social needs into medical treatment; and foster collaboration between health care institutions\, communities\, and cross-sector partners.
