SUMMARY:2021 Annual Conference Quick Take: Measuring What Matters to Older Adults
DESCRIPTION:Many existing quality measures target disease-specific\, intermediate clinical outcomes that are not meaningful to people with complex care needs and do not effectively evaluate what is important to people. With funding from The John A. Hartford Foundation\, The SCAN Foundation\, and The Gordon and Betty Moore Foundation\, the National Committee for Quality Assurance (NCQA) developed an approach to individualized measurement that focuses on goals oriented towards “what matters” for a person’s health and lifestyle. This Quick Take will provide an overview of the approach—known as person-driven outcomes—and discuss how incorporating patient-driven\, specific\, and measurable goals into clinical workflows can inform a new approach to measuring health outcomes for this population. \n \nThis session was designed by Grantmakers In Health and supported by Grantmakers In Aging.

DESCRIPTION:Cosponsored with Southern California Grantmakers \nNo one has been exempted from the mental health toll of the COVID-19 pandemic\, though experiences have differed across race\, gender\, income\, immigration status\, and disability. Children and youth are at particular risk\, as outlined in the December 2021 Surgeon General’s Advisory\, Protecting Youth Mental Health. The pandemic and its associated challenges\, along with preexisting stressors including racism\, gun violence\, climate change\, and increasing national polarization\, have taken a serious toll on the wellbeing of young people and their families.
