SUMMARY:Collective Impact: A High Performing Approach to Change
DESCRIPTION:Grantmakers around the country are embracing collective impact as a rigorous approach to bringing together a group of actors from different sectors to commit to a common agenda for solving a specific social problem. While collective impact can lead to meaningful results at scale\, it requires new mindset shifts and a long term commitment from all participating partners. The collective impact approach is particularly relevant in the health sector\, with new reforms underway and continuing complexity in the health space. \nThis webinar presented the collective impact model\, and explored the role of funders in creating and sustaining the collective process. Participants heard from health grantmakers implementing a collective impact initiative to reduce infant mortality by mobilizing\, facilitating\, coordinating\, and increasing engagement of multi-sector partners. Speakers included: John Kania and Jennifer Splansky Juster of FSG\, and Kathleen Holmes and Melissa Logsdon of Missouri Foundation for Health. \nRelated Content\n\nCollective Impact: A High Performing Approach to Change – PDF document\, 1156 KB

SUMMARY:Hospital Community Benefits and Community Health Improvement
DESCRIPTION:Cosponsored by Funders’ Network for Smart Growth and Livable Communities\, Grantmakers In Health\, the Health and Environmental Funders Network\, and The Sustainable Agriculture and Food Systems Funders\, and the Health and Housing Funders’ Forum. \nThe Affordable Care Act (ACA) establishes new community benefit requirements for tax-exempt hospitals and creates meaningful incentives for collaborative efforts to improve community health. This audioconference clarified the ACA’s hospital community benefit provisions\, including requirements for hospital-sponsored Community Health Needs Assessments\, and examined the promise (and potential pitfalls) of leveraging these rules to promote investments in community health. The discussion emphasized the critical role philanthropy can play in developing productive partnerships with hospitals and ensuring that the potential benefits of this important policy change are fully realized. Speakers included: Abbey Cofsky of Robert Wood Johnson Foundation\, Gary Cohen of Health Care Without Harm\, Jessica Curtis of Community Catalyst\, Chris Kabel of the Kresge Foundation\, and Kim VanPelt of St. Luke’s Health Initiatives. \nRelated Content\n\n2014 County Health Rankings and Roadmaps Overview – PDF document\, 2223 KB\nCommunity Catalyst Curtis Presentation – Microsoft PowerPoint presentation\, 1644 KB\nNEJM comm benefit 2013 Young et al – PDF document\, 564 KB\nRWJF Cofsky Presentation – Microsoft PowerPoint presentation\, 3589 KB\nHealing Communities and the Environment – PDF document\, 2713 KB\nRWJF Commission to Build a Healthier America – PDF document\, 1532 KB

SUMMARY:Monitoring the Impact of the ACA
DESCRIPTION:As the major open enrollment push draws to a close\, all eyes turn to monitoring the impact of the Affordable Care Act (ACA). Funders are interested in learning how the ACA will affect access\, affordability\, and outcomes. SUMMARY:Creating an Integrated Child Health System Using a Population Health Perspective
DESCRIPTION:Population health is a phrase that is becoming widely used among public health officials\, health care providers\, researchers\, policymakers\, and grantmakers. At its core is a focus on building sustainable linkages among health care systems\, public health agencies\, and the larger community to prevent the onset of chronic disease and improve health outcomes. \nDuring this meeting of grantmakers and other stakeholders\, we applied a population health framework to children’s health and addressed the question: What would an integrated child health system look like? We focused on innovative practices and policies and catalytic roles that grantmakers can play. \nThis meeting was made possible with support from Nemours. \nRelated Content\n\nArea Hotels – PDF document\, 252 KB\nAchieving Healthy Communities through Community Centered Health Systems (Fukuzawa) – PDF document\, 59 KB\nCommunity Pediatrics Policy Statement 2013 (Gorski and Kuo) – PDF document\, 673 KB\nIntegrator Role and Functions in Population Health Improvement Initiatives (Chang) – PDF document\, 223 KB\nPopulation Health in the Affordable Care Act Era (Stoto) – PDF document\, 317 KB\nCommunity Health Needs Assessments (Stoto) – PDF document\, 120 KB\nCreating an Integrated Child Health System Using a Population Health Perspective Agenda – PDF document\, 119 KB\nCommunity-Centered Health Homes\, Prevention Institute\, 2011 – PDF document\, 297 KB\nCommunity Health Centers Leveraging Social Determinants of Health\, Institute for Alternative Futures – PDF document\, 2986 KB\nClinical Integration_Haushalter – PDF document\, 1324 KB\nBest Babies Zone_Fehrenbach – PDF document\, 1383 KB\nCommunity Pediatrics_Gorski – PDF document\, 611 KB\nKresge_Fukuzawa – PDF document\, 1292 KB\nPopulation Health_Role of the CDC_Graffunder – PDF document\, 2685 KB\nTransforming Child Health System_Halfon – PDF document\, 3910 KB\nWhat Is Population Health_Stoto – PDF document\, 1342 KB
