SUMMARY:Scaling Evidence-Based Programs to Address Chronic Illness
DESCRIPTION:Cosponsored with Grantmakers In Aging \nIt is challenging to scale and sustain evidence-based health interventions\, especially those designed to meet the needs of populations experiencing multiple chronic illnesses. This webinar taught what the New York State Health Foundation and the National Council on Aging have learned about increasing the capacity and reach of promising programs for older adults\, how these lessons are applicable to other people with or at risk for chronic illnesses\, and the way this work can contribute to better outcomes for all. Speakers included Kathy Cameron and Jim Firman of National Council on Aging and Lourdes Rodriguez of The University of Texas at Austin. \nRelated Content\n\nScaling Evidence-Based Programs Webinar Slides – PDF document\, 1552 KB

SUMMARY:Collective Impact: Lessons Learned for HEAL Funders
DESCRIPTION:Collective impact has been a popular strategy employed to engage a wide spectrum of stakeholders to create broad social change. Many HEAL funders have used collective impact to address factors that contribute to the obesity epidemic and create health-promoting environments and communities. This webinar showed two experienced funders and their grantee partners about how they implemented an effective collective impact strategy in different contexts\, the lessons learned and challenges met along the way\, and the key takeaways they have for funders and communities looking to replicate their successes. Speakers included Amy Dyett of Colorado Education Initiative\, Hillary Fulton of The Colorado Health Foundation\, Peter Katzmarzyk of Pennington Biomedical\, and Michael Tipton of Blue Cross Blue Shield of Louisiana Foundation. \nAdditional resources: \n\nChallenge for a Healthier Louisiana Program Information\nChallenge for a Healthier Louisiana Blog Posts\nRequest copies of the Challenge for a Healthier Louisiana final and technical reports\n\nRelated Content\n\nCollective Impact Webinar Slides – PDF document\, 2260 KB\nHealthy Schools Collective Impact (HSCI) One Pager – PDF document\, 241 KB\nHSCI Partnership Principles – PDF document\, 38 KB\nHSCI Meeting Norms – PDF document\, 65 KB
