SUMMARY:Complex Care: Funding Big Problems on a Small Budget
DESCRIPTION:Cosponsored with Grantmakers In Aging \nThere is growing recognition that our current health care system often fails people with the most complex health and social needs. Promising approaches exist\, but Grantmakers\, particularly those funding at the local and regional levels\, often find the complexity of implementing and scaling new models to be challenging. This webinar taught strategies for funding improvement and innovation in complex care on a smaller budget. The discussion included an overview of The Blueprint for Complex Care (a joint project of the National Center for Complex Health and Social Needs\, the Center for Health Care Strategies\, and the Institute for Healthcare Improvement); case studies on how and why to support patient advocacy\, data analytics\, clinical models of care\, cross-sector communication\, unconventional partnerships\, and workforce development; examples of evidence-based programs that are ready for replication and scaling; and advice on designing sustainable business models and implementation plans. Speakers included Mark Humowiecki of National Center for Complex Health and Social Needs and Loren Pogir of Transforming Care Partners. \nWebinar Recording

SUMMARY:Sixth Annual Public-Private Collaborations in Rural Health Meeting
DESCRIPTION:2018 Agenda \nCosponsored with the Federal Office of Rural Health Policy and National Rural Health Association \nThe Federal Office of Rural Health Policy\, in partnership with Grantmakers In Health and the National Rural Health Association\, held the Sixth Annual Public-Private Collaborations in Rural Health meeting in Washington\, DC from June 7-8\, 2018. \nThe first Public-Private Collaborations in Rural Health meeting convened five years ago and brought together over 120 public and private funders and researchers with a focus on rural health. That initial event offered a series of presentations devoted to educating private funders about federal rural health-related programs and federal partners about the important work of the foundations in rural communities. Our partnership continues to grow and strengthen in its efforts to improve rural health through shared ideas\, research\, and combined funding\, among other activities. \nAs in each past year\, participants had the opportunity to engage with high-ranking program and policy experts within the government on issues and projects relevant to improving rural health and efforts supported by the partnership. \n\nMeeting Location\nThe meeting will be held from Thursday\, June 7 until Friday\, June 8 at the Liaison Capitol Hill in Washington\, DC. \nLiaison Capitol Hill Hotel\n415 New Jersey Avenue NW\nWashington\, DC 20001 \n\nRelated Content\n\nCollaborations in Rural Health Meeting Agenda – PDF document\, 405 KB

SUMMARY:2018 GIH Annual Conference on Health Philanthropy
DESCRIPTION:Agenda at a Glance \nThank you for attending the 2018 Annual Conference on Health Philanthropy\, the largest gathering of health funders in the country! Event details including session materials are available on the GIH App and app website. \n\nCONFERENCE THEME\nNavigating Currents of Change\nHow does your foundation or corporate giving program deal with change? Do you advance purposefully\, adapting your theory of change as needed? Are you doing the same things you’ve always done and hoped you won’t run aground? Or—worst of all—are you adrift? \nIn this time of dynamic social change\, every health funder must face staying relevant and effective. Like a choppy sea\, the challenges come from multiple directions: demographic shifts that affect community dynamics; increasing levels of social inequality; rapidly changing public policies; a health care system in transition; and new change management tools and techniques—to name just a few. \nThe good news is that from small communities to the national arena\, funders are developing successful strategies to stay abreast of change. The 2018 annual conference is a time to share these strategies\, identify best practices\, and discuss new priorities. Together\, funders can find ways to move forward with conviction\, providing the leadership that communities urgently need. \nReflections\nEach year\, GIH asks health funders to share their thoughts on our annual conference theme. This year\, we’ve asked evaluators from foundations to reflect on Navigating Currents of Change.
