SUMMARY:CalAIM\, Street Medicine\, and Homeless Services
DESCRIPTION:Cosponsored by Southern California Grantmakers and Funders Together to End Homelessness   \nThe California Department of Health Care Services is currently implementing California Advancing and Innovating Medi-Cal (CalAIM)\, a multiyear initiative intended to fundamentally alter how crucial health services are provided by reforming the state’s Medicaid program (Medi-Cal)\, particularly for the at-risk populations that philanthropy supports. CalAIM will require significant investments in network infrastructure\, capacity-building\, and patient outreach and engagement to be successful.   \nThis webinar focused on the intersection of CalAIM and street medicine—an emerging movement to provide direct care services to homeless individuals in the community wherever they are\, as opposed to requiring treatment in clinic or hospital settings. The philosophy behind street medicine aligns well with CalAIM’s intention to prioritize individuals with complex needs and integrate Medi-Cal more seamlessly with other social service programs. Participants learned about implementation\, the experiences of practitioners in the field\, and ways philanthropy can be a collaborative partner in the process.   \nParticipants learned:  \n\nWhy the connections between health and housing are so fundamental; \nHow the state and various partners define street medicine; \nHow CalAIM can support street medicine\, and where additional clarity or support may be needed; and \nWhere philanthropy can be most useful\, especially regarding coordination of care\, cross-sector collaboration\, and research into true costs and best practices.   \n\nPowerPoint Presentation: \n\nHealth Funders Group Meeting: CalAIM\, Street Medicine\, and Homeless Services\n\nResources: \n\nStreet Medics Help Formerly Homeless Woman Rebound Video\nStreet Medicine Institute\nAll Plan Letter from State of CA Dept of Health Care Services\nCHCF Issue Brief on the All Plan Letter\nCHCF Work in this Space\nStreet Medicine Bakersfield\nStreet Medicine Teams Tackle the Homeless Health Care Crisis\, from CalMatters\n\nRecording:

SUMMARY:Multisector Plans for Aging: A Global Perspective
DESCRIPTION:Cosponsored by Grantmakers In Aging \nAcross the United States\, Multisector Plans for Aging are driving public-private partnerships at the highest level of state government. Speakers included Peter Kaldes of American Society on Aging and Vijeth Iyengar of AARP’s Office of International Affairs. \nReference Links: \nUN resolution on the Decade https://undocs.org/en/A/RES/75/131   \nRead the plan for the Decade: https://www.who.int/initiatives/decade-of-healthy-ageing  \nCheck out the Decade Platform: https://www.decadeofhealthyageing.org \nData Portal: https://www.who.int/news/item/01-10-2020-who-launches-portal-for-global-data-on-the-health-and-well-being-of-older-people  \nThe links between the Decade and COVID 19\, the SDGs or climate change in the first of our connection series https://www.who.int/initiatives/decade-of-healthy-ageing/connection-series   \nSubscribe to Decades Newsletter: https://who.us3.list-manage.com/subscribe?u=c7943277461ee4157547221f2&id=4488fab1b7   \nhttps://www.aarpinternational.org/  \nInnovation and Leadership in Healthy Aging (aarpinternational.org) \nhttps://www.chcs.org/project/multisector-plan-for-aging-learning-collaborative/  \nhttps://www.asaging.org/on-aging  \nRecording:

SUMMARY:Supporting Communities to Leverage Federal Food and Nutrition Opportunities
DESCRIPTION:Even before last year’s White House Conference on Hunger\, Nutrition\, and Health\, the Biden administration made significant investments to expand and improve food access and nutrition. Despite these efforts\, many communities that would benefit most face significant barriers to leveraging these opportunities. Leaders from the field explored the current federal options to support food access and nutrition and how philanthropy can better help support communities. Speakers included Elizabeth Atwell and Ellie Bomstein of Wallace Center at Winrock International and Ben Feldman of Farmers Market Coalition.  \nResources: \nIncreasing Access to USDA Funding \nRecording:
