Care for the Seriously Ill in Low-Income Communities

Even in the best of pre-COVID times, access to high quality care for those with serious illnesses is challenging. For those who are homeless, living on low incomes or in disenfranchised rural communities, or are otherwise vulnerable, treatments and supports that effectively manage symptoms, improve quality of life, and reduce hospitalization are virtually nonexistent. This session explored two trailblazing programs that are addressing the care needs of the most vulnerable and seriously ill, how each program works, how palliative care can be effective in addressing disparities and social determinants of health, and how grantmakers can play a vital role. Speakers included Janet Bull of Project ECHO - Four Seasons and Will Kennedy of CareOregon and Housecall Providers.
