If Not Now, When? Integrating Social Care into the Delivery of Health Care

Addressing the social determinants of health has become a major focus of health care organizations, which are determining how to best partner with community-based organizations in order to improve outcomes for patients of all ages. The reality is that health systems, payers, social services, and community-based organizations must learn how to come together to address social and behavioral determinants of health. Participants learned key recommendations from the National Academies of Sciences, Engineering, and Medicine report, “Integrating Social Care into the Delivery of Health Care: Moving Upstream to Improve the Nation’s Health.” Speakers discussed how social services can—and in some settings are—being integrated into clinical care; the infrastructure needed to make it happen; and how foundations might play a role. Speakers included Kirsten Bibbins-Domingo of University of California: San Francisco, Robyn Golden of Rush University Medical Center, and Dawn Simonson of Metropolitan Area Agency on Aging.
