Taking a Collective Approach to Reducing Diabetes Disparities

Type 2 diabetes rates continue to skyrocket across the United States, and people of color, low-income individuals, rural communities, and older adults remain disproportionately affected by the disease. This highlighted lessons learned and promising practices from Together on Diabetes, an initiative designed to reduce disparities in type 2 diabetes through strategic partnerships, community-based supportive services, and patient-centered models of care. Speakers included: Richard Crespo of Marshall University, Patricia Doykos of Bristol-Myers Squibb Foundation, David Gould and Fredda Vladeck of United Hospital Fund, and Kim Prendergast of Feeding America.
