Community-Based Health Systems: Using Housing as a Platform

Foundations are investing in a range of strategies to improve health through housing, including housing-based service models. This emerging approach has been shown to reduce Medicare spending significantly while improving health and expanding access to care. This webinar explored how housing-based service models address the social determinants of health, promote population health, and advance health care systems change. This webinar taught about two successful housing-based service models in Vermont and Oregon and recent research that evaluated the health impact of this model. Speakers included: Abigail M. Crocker of the University of Vermont, Nancy Eldridge of the National Center for Healthy Housing, Howard Klink of Housing with Services LLC, and Amanda Saul of Enterprise Community Partners.
