Community-Based Health Systems: Using Housing as a Platform

Foundations are investing in a range of strategies to improve health through housing, including housing-based service models. This emerging approach has been shown to reduce Medicare spending significantly while improving health and expanding access to care. This webinar explored how housing-based service models address the social determinants of health, promote population health, and advance health care systems change. This webinar taught about two successful housing-based service models in Vermont and Oregon and recent research that evaluated the health impact of this model. Speakers included: Abigail M. Crocker of University of Vermont, Nancy Eldridge of the National Center for Healthy Housing, Howard Klink of Housing with Services LLC, and Amanda Saul of Enterprise Community Partners.
