It is challenging to scale and sustain evidence-based health interventions, especially those designed to meet the needs of populations experiencing multiple chronic illnesses. This webinar taught what the New York State Health Foundation and the National Council on Aging have learned about increasing the capacity and reach of promising programs for older adults, how these lessons are applicable to other people with or at risk for chronic illnesses, and the way this work can contribute to better outcomes for all. Speakers included Kathy Cameron and Jim Firman of National Council on Aging and Lourdes Rodriguez of The University of Texas at Austin.
