The North Carolina Department of Health and Human Services has launched a Healthy Opportunities initiative to address the social determinants of health (SDOH) with an initial focus on housing stability, food security, transportation access, and interpersonal safety. The multifaceted initiative includes the creation of an interactive statewide map of SDOH indicators, the development of standardized screening questions to identify and assist patients with unmet health-related resource needs, the establishment of a statewide coordinated care network (NCCARE360), and the creation of innovative Healthy Opportunities Pilots to test and evaluate the impact of providing select evidence-based, nonmedical interventions to high-need Medicaid enrollees.
