The Health Foundation of Central Massachusetts: January 2025
2025 Activation Fund grant opportunity is now open, which provides capacity-building support for organizations working to address the community-identified health needs of Central Massachusetts residents and workers. With grants typically ranging between $60,000 and $125,000, the Activation Fund supports discrete, one-year projects in Central Massachusetts aimed at helping organizations advance to their next level of capacity and effectiveness that can be sustained over time.
Merck Foundation: December 2024
Merck Foundation has launched a new initiative – the Collaborative for Equity in Cardiac Care to advance equitable access to high-quality, culturally responsive cardiac care for people living with heart conditions in underserved United States communities. The foundation is committing $17 million over five years to support the development and implementation of innovative, comprehensive programs for cardiac health care across the country as well as promote cross-sectoral collaborations to address barriers to care associated with social drivers of health.
Vitalyst Health Foundation: October 2024
Vitalyst Health Foundation launched its Systems Change Grant request for proposals. The Systems Change Grants support coalitions in implementing their desired systems change. By the end of the grant period, coalitions will have either fully implemented or made significant advancements toward their systems change project.
Horizon Foundation: September 2024
The Horizon Foundation opened its 2024 Housing Justice Request for Proposals, seeking organizational partners to co-create solutions that remove barriers to obtaining and sustaining affordable, high-quality rental housing and home ownership. The first phase of co-creation will focus on three housing priorities over the next two years: expanding renter protections, sustaining homeownership, and strengthening systems to advance housing justice.

The Imagine Nonviolence: Expand the Reach initiative is designed to support innovative and collaborative programs working to reduce the risk of firearm-related injury and death among pregnant people, children under age five, and older adults in western and central New York.
Foundation for Opioid Response Efforts: July 2024
The Opioid Crisis Innovation Challenge marks an expansion of FORE’s Innovation Program, which was launched in 2022 to support projects that combine strategies from diverse fields and engage multidisciplinary teams in tackling some of the most intractable problems associated with the nation’s opioid-related addiction and poisoning crisis.

The Special Initiatives grant program provides organizations with a one-time grant of up to $50,000 to pilot or launch a new project over a one-year period. This grant is intended to fund projects that empower communities to advance health equity.
