Philanthropy @ Work – Grants and Programs – February 2017

Williamsburg Health Foundation (VA)

In 2016’s second and final round of grants, the Williamsburg Health Foundation awarded over a million dollars to 15 different organizations – bringing the year’s grants total to more than $4.5 million. Initiatives include:

  • Child and Family Connection’s Neurofeedback Counseling Program which teaches self-regulation by showing patients a real time EEG feed of their brain’s activity.
  • The Peninsula Metropolitan YMCA will host a Diabetes Prevention Program at the RF Wilkinson YMCA for adults diagnosed with prediabetes. The course connects people with the coaching, information, and tools to maintain a healthy lifestyle.
  • The Doorways offers lodging to Medical College of Virginia patients and their families in Richmond, Virginia.
  • Olde Towne Medical and Dental Center was awarded $450,000 to support its operations. An additional $50,000 will fund a program to connect uninsured or underinsured hospital patients with a nurse for follow-up recommendations and referrals to both Olde Towne and the Lackey Clinic.

Grants also include $78,400 for access to healthy food; $41,000 to increase active living; and $194,000 for mental health services.

Sunflower Foundation (Topeka, KS)

The Sunflower Foundation board of trustees has approved a statewide $120,000 pilot project focused on supporting community-based strategies to address food access needs in rural areas, such as strengthening local grocery stores. The pilot, called Project HERO – Healthy Eating: Rural Opportunities, will initially fund planning efforts in eight communities, totaling $120,000. The planning grants are intended to provide community teams with the resources and technical assistance needed to explore and develop sustainable strategies for increasing access to healthy foods.

The eight HERO grantees include:

  • Western Prairie Food, Farm & Community Alliance, working with the Western Prairie Resource, Conservation & Development District, representing eight counties in northwest Kansas.
  • GROW Hodgeman, working with the Hodgeman Co. Economic Resource Development Council, focusing on Jetmore and Hanston.
  • Allen County GROW, working with Thrive Allen County, focusing on Moran.
  • Harvey County Food & Farm Council, working with Harvey Co. Health Department, focusing on Harvey County as a whole.
  • Southern Marion County Healthy Partnerships, working with Peabody Main St. USA, focusing on Peabody and Burns.
  • Eat Well Crawford County, working with Crawford Co. Health Department, focusing on the eight towns in Crawford County without grocery stores.
  • Community Enhancement Foundation Of Plains, focusing on Plains (Meade County).
  • St. John Task Force for Grocery Store Access, working with Stafford Co. Economic Development, focusing on St. John.

Sisters of Charity Foundation of South Carolina (Columbia)

The Sisters of Charity Foundation is partnering with the South Carolina Association of Nonprofit Organizations (SCANPO) to design a comprehensive, statewide learning and development program focused on excellence in nonprofit practice. This new grant funded program will provide opportunities that build on the foundation’s long-standing Carolina Academy program that trained more than 1,500 nonprofit leaders and practitioners over the last nine years.

SCANPO, a nonprofit membership organization for South Carolina, will enhance and expand the capacity building training offerings to all nonprofits across the state to include members as well as nonmembers. The new model will be entirely based on research and input provided by nonprofit leaders.

Moses Taylor Foundation (Scranton, PA)

The Moses Taylor Foundation awarded $588,110.56 to 14 organizations that advance its strategic priority of promoting health and wellness throughout Northeast Pennsylvania.

The full list of 2016-2017 second quarter grants:

  • Northeast Regional Cancer Institute: $100,000 to support the Community Based Cancer Screening Navigation Program.
  • Sullivan County Dental Clinic: $100,000 to relocate and expand the Sullivan County Dental Clinic.
  • Wyoming Valley Catholic Youth Center: $75,000 to reopen the child care center at St. Mary’s Church.
  • Penn State Worthington: $74,110.86 to purchase a high-fidelity human mannequin simulator that can be safely used for obstetric training of birth management from pre to postnatal care.
  • Employment Opportunity & Training Center (EOTC): $56,826 over two years for the purchase and implementation of a new web-based outcomes management system that will bolster case management, program evaluation, data-based decisionmaking, and communications.
  • Boys & Girls Clubs of Northeastern PA: $50,000 to support its after-school program—a safe and healthy environment where children and teenagers have fun, eat healthy food, and participate in activities that are preventative, nutritional, and physical in nature.
  • The Arc of Northeastern PA: $35,750.70 to purchase automated external defibrillator (AED) devices for its group homes, summer camp, and adult day and senior programs.
  • Catholic Social Services of Lackawanna County: $20,000 to assist with general counseling program operating expenses.
  • Wyoming County United Way: $20,000 to support the Children’s Produce Market which provides free produce and other nutritious food items to food-insecure households.
  • Family Service Association: $18,365 to update the 211 Northeast PA website that include implementing greater search capacity of resources, designing a chat room for people who are hesitant to text personal issues, and establishing a system for updating provider information.
  • Partners in Family and Community Development: $18,058 to support the Strengthening Families Program.
  • Institute for Public Policy & Economic Development: $10,000 to support the 2017 Regional Indicators program.
  • Friends of the Poor: $5,000 to support the organization’s annual community Thanksgiving meal event in Scranton.
  • Scranton School District: $5,000 to update strength and fitness training equipment at West Scranton High School.

Pottstown Area Health & Wellness Foundation (PA)

The Pottstown Area Health & Wellness Foundation has awarded $945,786 in grant requests during the fall grant round of 2016 to 39 nonprofit organizations, schools, and municipalities. The following is a representative list of organizations receiving health related funding:

  • Montgomery County Community College Foundation: Dental Sealant Project
  • Developmental Enterprises Corporation/Pottstown Training Center: Healthy Eating, Healthy Living
  • Pottstown Cluster of Religious Communities: Promoting Health and Stability in Our Community
  • Wellness Council of Boyertown: Supporting Health and Wellness in our Community
  • Family Services of Montgomery County: Automatic External Defibrillator for the Pottstown Office of Family Services of Montgomery County

Richmond Memorial Health Foundation (VA)

Richmond Memorial Health Foundation announced $250,000 in grants to five nonprofit organizations supporting improved health and health care access in the Richmond, Virginia region. These awards bring total RMHF commitments to $890,000 for the current grant cycle, which began in July 2016.

Recently approved grants include:

  • Institute for Public Health Innovation, $80,000 over one year to support community health workers in Resource Centers in each of Richmond, Virginia’s public housing communities.
  • Free Clinic of Powhatan, $50,000 over one year in operating support to provide health services to uninsured and underinsured low-income adult residents of Powhatan County, Virginia.
  • Access Now, $45,000 over one year in general operating support to provide access to specialty medical care for uninsured patients of participating safety-net clinics.
  • Greater Richmond SCAN, $35,000 over one year to support coordination of the region’s Trauma Informed Care Network
  • Rx Partnership, $20,000 over one year to increase access to medication for Virginia’s vulnerable populations and serve as a resource to the organizations that support these populations.

Also in this grant cycle, the board approved the following grants:

  • CrossOver Healthcare Ministry ($100,000)

Horizon Foundation (Columbia, MD)

As a core part of its mission to improve health and wellness for people who live or work in Howard County, Maryland, the Horizon Foundation awarded nearly $1 million in grants in 2016. The grants support programs that address community needs and advance the foundation’s strategic priorities to increase access to quality, affordable health care, and to promote healthy lifestyles.

This year the Horizon Foundation awarded eight new Strategic Health Initiative Grants:

  • Howard County Government: $250,000 to pilot a bikeshare program in Columbia, Maryland with the goal of increasing transportation options that increase daily physical exercise. 
  • Howard County Health Department: $149,453 to enhance health care access for youth by expanding school-based behavioral health and telepsychiatry consultation, provided in partnership with the Department of Psychiatry at the University of Maryland School of Medicine.
  • Family and Children’s Services of Central Maryland: $100,000 over two years to expand comprehensive behavioral health care by becoming licensed as an outpatient mental health clinic, adding child psychiatry, and expanding capacity.
  • Way Station Inc.: $95,000 to provide residential crisis beds for individuals, regardless of income, where they can receive intensive mental health support services from licensed professionals in a therapeutic setting.
  • Humanim: $60,000 to study the feasibility of increasing access to behavioral health services in the county by expanding the number of providers accepting patients covered by Medicaid.
  • Community Action Council of Howard County: $45,000 to support the Healthy Families, Healthy Children program.
  • Grassroots Crisis Intervention Center: $28,500 to expand mental health education that will help individuals learn how to identify, understand, and respond to signs of mental illness and substance use disorders.
  • Howard County Department of Fire & Rescue Services: $10,000 to assess the feasibility of a mobile integrated health program that will help reduce operational strain on emergency care resources and improve care for patients with complex medical conditions.

Maine Health Access Foundation (Augusta, ME)

The Maine Health Access Foundation (MeHAF) has awarded $240,000 to five organizations around the state to develop cross-sector collaborative approaches to improve health and health care access in Maine’s rural communities. Three “Catalyst” grantees will initiate new collaborative efforts, while two “Acceleration” grants will support existing collaborations to pilot programs that test novel ways of delivering essential health services.

Examples of grantee activities include: establishing or bringing new partners into a community coalition to focus on the rural health care system, building capacity through targeted operational and organizational integration, identifying and addressing service gaps, connecting this work with ongoing economic development planning, and educating and engaging the community.

  • Health Access Network, Inc. (HAN)—Rural Healthcare Integration Planning Collaborative ($30,000)
  • Mayo Regional Hospital – Mayo Community Outreach—Piscataquis Healthcare Collaborative ($30,000)
  • Pines Health Services – Aroostook County Health Network (ACHN)—Continuum of Care Coordination in Aroostook County ($30,000)

Acceleration Grants:

  • Central Maine Healthcare Corporation—CMH Rural Health Transformation Initiative ($75,000)
  • Healthy Acadia—Washington County Substance Use Response Initiative ($75,000)

John A. Hartford Foundation (New York, NY)

The John A. Hartford Foundation approved four new grants totaling $5.5 million to accelerate the move to age-friendly health systems that measurably improve care for older adults while lowering costs.

  • Institute for Healthcare Improvement: Age-Friendly Health Systems ($3,190,452 for 42 months)
  • American Geriatrics Society: Geriatric Orthopedic Hip-Fracture Co-Management Implementation ($1,399,263 for three years)
  • Education Development Center: National Co-Laboratory to Address Elder Mistreatment ($774,984 for two years)
  • Institute for Healthcare Improvement: Development of the High-Need, High-Cost Playbook Version 2.0 ($180,838 for one year)

George Family Foundation (Minneapolis, MN)

The George Family Foundation renewed its commitment to women’s leadership development and access to healthy food for low-income people with diabetes in its latest grant round, which supports the Women’s Foundation of Minnesota’s Wenda Weekes Moore Internship and Reatha Clark King Fellowship Program, Second Harvest Heartland’s FOODRx program, and other select organizations.  The foundation awarded $1,806,500 in this grant round, out of an annual giving budget of more than $3.8 million. Other health related grants include: 

  • Academic Consortium for Integrative Medicine & Health  ($100,000) 
  • Global Forum on Innovation in Health Professional Education/National Academy of Sciences ($100,000)
  • Penny George Institute Foundation  ($100,000)
  • Mind and Life Institute  ($25,000)

  • Children’s HeartLink  ($15,000)
  • Walk-In Counseling Center  ($15,000)

Foundation for a Healthy High Point (NC)

In 2016, the board of directors for the Foundation for a Healthy High Point approved $5,449,757 in grant awards to 21 organizations. These funds support projects in areas of teen pregnancy prevention and early intervention, behavioral health, and other services. To date, the foundation has approved $6,769,863 in grant awards.

The California Wellness Foundation (Woodland Hills, CA)

The California Wellness Foundation announced nearly $6.3 million in grants made during the fourth quarter of 2016. Thirty-three grants were approved to organizations focused on advancing wellness in California. A number of these grants will help address some of the most urgent issues facing California. Highlights include grants to ensure access to health coverage for immigrant communities, to support fiscal and policy analysis and public education to inform health related policy and budget decisions, to strengthen community clinics, and to advance social justice campaigns and direct services work aimed at improving race relations and preventing violence in communities. Since its founding in 1992, California Wellness has awarded 8,531 grants totaling more than $960 million.

Blue Cross Blue Shield of Michigan Foundation (Detroit)

The Blue Cross Blue Shield of Michigan Foundation recently awarded three research grants to Michigan researchers. Among the grantees are:

  • Michigan State University, Paul D. Stein, M.D. ($72,170) to determine if mortality can be decreased in unstable patients with pulmonary embolism with the use of vena cava filters by comparing mortality for those who received a filter versus those who did not.
  • University of Michigan, Sanjay Saint, M.D. ($69,798) to examine current use of infection prevention practices by acute care hospitals and assess trends in the use of practices by both Michigan and non-Michigan hospitals since 2005.
  • Central Michigan University, Karen Lomond, Ph.D. ($47,150) to determine how people suffering from low back pain carry their weight in certain positions.

The Episcopal Health Foundation (Houston, TX)

The Episcopal Health Foundation (EHF) has approved $3.2 million to benefit 17 new grant partners working to take an active role in solving the underlying cause of poor health for Texans. With these latest grants, EHF’s board of directors awarded 122 grants in 2016 worth more than $16.2 million to organizations working across 57 Texas counties to improve health for the poor and those with the least resources.

EHF’s latest grants are designed to help bolster the ability of nonprofits and medical providers to have a larger and more sustained impact on the overall health of those they serve.

New grants announced include:

  • Northwest Assistance Ministries ($300,000)
  • Patient Care Innovation Center ($250,000)
  • Austin Interfaith Sponsoring Committee, Inc. ($150,000)
