The Center at Sierra Health Foundation in partnership with the California Department of Health Care Services announced $12 million in available funding and technical assistance through the Asthma Preventive Services Project. This funding will support a statewide network of organizations offering culturally and linguistically appropriate asthma home visiting services to individuals with poorly controlled asthma, with a focus on low-income communities and communities of color who have disproportionate rates of asthma.
Multiyear grants of up to $500,000 per project will fund eligible nonprofit organizations, community-based health care providers, health departments, and managed care organizations providing asthma home visiting, education, and environmental mitigation services, with a focus on the following outcomes:
- Improvement in asthma self-management
- Decreased exposure to environmental triggers
- Improved asthma outcomes
Applications are due April 28, 2020 at 1 p.m. Funding will support efforts through 2023.
