Ending Disparities in Diabetes Care

The Merck Foundation recently announced a new initiative that aims to promote health equity for communities disproportionately affected by diabetes. Bridging the Gap: Reducing Disparities in Diabetes Care is a five-year initiative that will build multi-sector collaborations and support evidence-based practices that link clinical care with services outside the health sector.

The initiative will provide $16 million in funding over five years to programs that target the upstream factors that influence diabetes outcomes including access to healthy foods and places for physical activity. Bridging the Gap also will work to improve the delivery of high-quality care for vulnerable and medically underserved populations.
